WHITE BLOOD CELLS Options

Hi,
Im a bit worried about my white blood count, it has gone low before but now it is at its lowest of 2.9. Has anyone had a lower white blood count?

I have had my first MTX injection today, the nurse did not want to let me go ahead with it, but the doctor said it would be ok because it's my RA thats killing the cells and not the MTX.

Hi Gill

As Jean says I have had various problems with low white cell counts and it can become quite a serious issue if not dealt with quickly.

The lower the white cell count, and in particular the neutrophils, the less resistance you will have to infection. I suffered for 5 months last year with ongoing infection and neutropenia which was caused by a reaction to an increased dose of methotrexate (I had been taking it successfully for years prior to this increase). Normally '4.0' is regarded as the lowest acceptable level for the white cell count and 2.9, I would have thought (and in my own experience!) would suggest not proceeding with methotrexate until the count improves. The minimum acceptable level for neutrophils is '2.0', anything less is regarded as Neutropenia. What was your neutrophil count when the WCC was 2.9?

Methotrexate works by inhibiting the activity of the immune system and consequently reducing/slowing down the rate of inflammation. The RA is producing the inflammation that the methotrexate is working to control, so I'm not sure I follow your doctor's thinking!

Hi Gill,

When I was on 30mg MTX I had continually low white blood cell counts. I was told by my rheumy nurse that I could stay on MTX provided my white cell count didn't go below 3 (although it was 2.8 a couple of times and they left me on it) and my neutrophils didn't go below 1. I would doudble check with your nurse to be on the safe side.

Thanks your replies.

Lyn, when my WBC was 2.9 my neutrophils were 1.69, I know this is a bad, but because the doctor said I could go ahead with the MTX injection I was given the first one on Wednesday. I feel like I am waiting for a time bomb to go off (meaning an infection).

My next blood test will be in two weeks and i hope and pray that both results will be better. I don't see the doctor at the hospital until March, If my results are worse on my next test I think I will try an get an appointment before. Also I have gone on the waiting list to have an operation on my toes, I think that will be a no no if things don't improve.

Mary, a few years ago I saw a blood specialist, he told me if my neutrophils went below 1 it would be time to worry and it was on his say so that I have been allowed to keep taking the MTX when my WBC was low.
